TRACKING PATIENT OUTREACH & TEAM CHANGE-MANAGEMENT
A regional rural health care agency was awarded a special grant to implement a wrap-around system of care for patients with multiple chronic conditions. Walter Sive Consulting LLC developed a customized tracking system for the patient population, whereby multiple users updated a secure central database.
Teams were also coached on best-practices for monitoring team change management by training them on in-process measures. This engagement facilitated the development of operational competencies that now enable the agency to take on other strategic reforms.
DEMAND & EXPENSE FORECASTING
A large health care insurer was experiencing unreliable expense forecasts from a large administrative division with many departments and scores of independently contracted application developers. Walter Sive Consulting LLC engaged in a two-pronged effort--extensive interviews to understand the expense drivers of particular departments and development of a sophisticated forecasting model. The latter enabled incorporation of over a hundred statistical, mathematical and judgmental variables.
The new forecasting tool provided not only a foundation for much more accurate forecasting, but also a tool that could be replicated for use by other divisions.
