POPULATION REGISTRY AND POPULATION REPORTING
POPULATION REGISTRY AND POPULATION REPORTING
A large regional health care insurer, covering over one million lives in medicaid programs, was floundering in the development of analytics and related data development tools.
A specific initiative to integrate care among behavioral health, community health and traditional physical health providers needed tracking tools to facilitate patient prioritization and related analytics to judge the assess progress in the population health initiative.
Walter Sive Consulting LLC developed a simple and easy to deploy population registry and related analytical systems to monitor the select patient population. These tools provided a foundation for continuing development of population health initiatives.
